Meaningful Use and PQRS Reporting Assistance

Direct Secure Messaging utilizes the Internet to protect sensitive patient information while helping practices achieve Stage 1 Meaningful Use. Direct is an encrypted, HIPAA-compliant messaging service that supports secure electronic communication between physicians and other health care providers, as well as office staff who deal directly with patient care. Providers can securely exchange clinical summaries, referrals, medical images, and transition of care documents through their Direct email addresses. Unlike traditional email, the content of a secure email and all associated attachments are sent through and received only by the intended recipient in a secure Clinical Message Portal. Direct communicates across organizational borders with and without an Electronic Health Record (EHR) system. Providers can achieve Meaningful Use by sharing clinical data through Direct. Direct also serves as a gateway to the CRISP Health Information Exchange.

Privacy and Security

According to the Health Insurance Portability and Accountability Act (HIPAA), any organization processing or maintaining healthcare-related information must demonstrate its compliance with security regulations for electronic protected health information (ePHI). Under the HITECH Act, covered entities, including healthcare providers, must implement policies that address the use and disposition of electronic PHI and the hardware or electronic media on which it is stored.

Health Information Exchange

The ZaneNet Connect team can help you determine whether your practice is ready for the patient-centered medical home (PCMH) transformation and assist you if you choose that process. In a PCMH practice, a team of health professionals, guided by a primary care provider, provides continuous, comprehensive, and coordinated care to patients throughout their lives. The PCMH collaborates with other qualified professionals as necessary.

Practices that provide first-contact, continuous, comprehensive, whole-person care for patients across the practice are eligible to participate. Primary care physicians represent most of the recognized practices – including family practitioners, general practitioners, internists and pediatricians.

Whole person care includes patient self-management support and emphasizes a spectrum of care needs: routine and urgent care; mental health care; and assistance and support for improved health habits and health care decisions. Preventive care is a key component.

Telehealth Support

Care Coordination

ZaneNet Connect has been contracted by CRISP, the DC Health Information Exchange, to assist providers to sign up for the Encounter Notification Service (ENS), a service which enables physicians to receive real-time alerts when a patient is hospitalized. The service is offered in partnership with participating hospitals at no cost to ambulatory providers. Benefits of participating in ENS include:

  • Receive real-time notification of your patients’ hospital visits (admit, discharges, or ER) for active patients in your practice
  • Proactively coordinate your patients’ care and schedule any necessary follow-up treatment or visits
  • Receive service FREE if a provider affiliated with a participating hospital

ZaneNet also works with several innovative projects in both DC and Maryland to assist health information exchange and CCD exchange between institutions such as long-term care facilities, care management organizations, clinics and hospitals.

As an MSO, our mission according to the MHCC is to do the following: A State Designated management service organization (MSO) shall make available to health care providers health information technology, such as electronic health records, health information exchange, and/or telemedicine.  MSOs shall provide services to health care providers that support the providers in improving the patient experience of care, improving the health of the population, and reducing health care costs.
